For the Custom ID we need the following information:
1. NAME (Last, First)
2. TITLE: (example: FIELD OFFICER / SPECIALIST / ADMINISTRATOR / BIO-CHEMIST / ENGINEER / FIELD AGENT / DIRECTOR)
3. SEX, HAIR, HEIGHT: (example: SEX:M/F. HAIR: BLK/RED/BLN/WHT/BRN. HEIGHT: Feet, Inches)
4. ISSUE DATE/EXP. DATE: (example: 2022JUL25/2026JUL24)
5. CODE: (example: SKJ 08U7441 No more than 10 characters)
6. BARCODE: (example: 08/30 /99 508 13/FD/97 1336. Please follow this format)
7. ID Photo: (Please provide a photo with a resolution greater than 300 dpi, a plain white background is preferred.)

The custom ID is fully customized for you, so we do not accept refunds on this particular item since it is custom made. We are not responsible for spelling errors or mistakes.
